A Special Incident is an unusual adverse event in the life of an individual receiving Regional Center services.

Main Categories for Reporting

• Serious Injuries requiring medical attention
• Medical needs, accidents, injuries, or other (medication refusal or medication documentation error only)
• Suspected abuse or exploitation
• Suspected neglect
• Unauthorized absence
• All hospitalizations, including urgent care or emergency room visits
• Victim of crime
• Aggressive acts
• Death

Special Incidents must be reported to San Andreas Regional Center by both phone and email within 24 hours of the incident. This allows for processing and submission to DDS within the 48-hour timeframe outlined in Title 17 Section 54327. Updates must be submitted by adding follow-up details including appointments and the After Visit Summary with the date of new information; do not delete any information from the initial SIR form submitted.

Please attend SARC SIR training annually to stay current with changing requirements and expectations. Note: Reporting to DDS of an individual with a positive COVID test is no longer required. You may send a SIR to SARC or notify the Service Coordinator by email.
